Appointments
To make an appointment call the Orthopedic & Fracture Clinic at (503) 214.5255, between 8:30 a.m. and 5:00 p.m. The appointment secretary will ask about your problem in order to schedule enough time for the doctor to care for you. Please call for appointments as far in advance as possible. If you cannot keep an appointment, please notify the clinic promptly. Sometimes emergencies interfere with doctor's appointment schedules. Calling shortly before your appointment will let you know if your doctor is on time and may cut down on unnecessary waiting for you.

Referrals
Have you been referred by another doctor to us? Some insurance plans require a referral form from your primary care physician before you can see a specialist for your condition. If you have this type of insurance plan, we will need to receive a copy of the referral form from your PCP before a visit can be scheduled.
Our payment policy
All accounts are due and payable within 30 days after you receive your first statement. Patients without insurance will be required to make payment at time of their office visit. If your financial circumstances prevent you from meeting this policy, or if you have questions regarding your statement, please call (503)214-5200 and ask for an account representative.
Surgery Deposits
If surgery is required, your insurance benefits will be verified by our staff and you will be required to pay any deductibles and coinsurance before the surgery date.
Your Medical Records
Information about your medical care at the Orthopedic & Fracture Clinic becomes part of your medical record, which is protected by strict confidentiality. If you would like us to release information about your health care, we will ask you to sign a form authorizing us to do so. We will then be happy to transfer that information to anyone you choose.
To refill your prescriptions
Your doctor must authorize the pharmacist to refill your prescription, so it's a good idea to call the pharmacy a day or two before you need the prescription. Prescriptions can be refilled only during regular office hours. |
